Instructions
Fill
out all information included in the payment page fields. After
your payment is made a Thank You page will appear. This holds
the link to the Consultation Questionnaire. Click the link to
access it. Follow the directions and fill out all fields of the
questionnaire. This must be completed and submitted to begin
your Consultation Analysis.
In
the last field of the questionnaire put your preferences for
appointment times. You will be contacted within 7 business days
to make the appointment.
